Nomination Form
We welcome nominations from students, faculty, and alumni of professors and teaching assistants. Submitting a nomination is straightforward:
1. Please review the President's University-Wide Teaching Award criteria and nomination information. You can read these here.
2. Please review the entire form. Do not fill it out yet.
3. Please assemble the letters of nomination and support documentation described below
4. Return to this form to fill in the details, upload the letters, CV, summary of teaching evaluations, and statement of teaching/learning philosophy, and submit the nomination.

The deadline for nominations is January 25, 2019.

NOTE: Please pay close attention to the word limits on all documents. To ensure equal treatment, additional documentation or documentation over the specified word limits will not be considered.
If you can only obtain a paper copy of the curriculum vitae, please contact the secretary of SCOTL to have it scanned for inclusion in the nomination.
